Required Documentation

Proper identification must be submitted with your license application. The Ministry requires three pieces of ID and will only accept the following. Please bring them with you and we will make photocopies.

	Canadian Birth or Baptismal Certificate
	Ontario Driver's License
	Social Insurance Card
	Canadian Passport
